Short answer

If bone-strengthening therapy does not relieve persistent bone pain, contact your oncology team for a fresh assessment and a revised pain plan.

This answers one question. It is not a diagnosis and does not replace an assessment by a licensed doctor.

What continued pain can mean

Bone-strengthening medicines are used to support bone health and reduce problems related to weakened bone, but they may not address every source of pain. Pain can come from an irritated joint, muscle strain, a fracture, nerve involvement, treatment effects, or changes in the cancer. The timing, exact location, movement triggers, night-time pattern, and response to each dose help the team work out what needs attention. Ongoing pain does not by itself show why it is happening or mean that the treatment has failed.

Practical next steps

Keep taking or pausing the medicine only as directed; do not add supplements or change doses without checking. Record when pain starts, where it travels, its severity in everyday terms, and whether walking, lifting, coughing, or resting changes it. Tell the team about missed doses, new medicines, dental symptoms, constipation, drowsiness, or nausea. They may review scans or blood tests, check for an injury, adjust pain relief, or involve palliative-care and rehabilitation specialists. Protect a painful area from falls and heavy loading until you have specific advice.

When to seek urgent help

Seek urgent medical help for sudden severe pain, a limb that looks different after a minor movement, inability to stand or walk, new weakness, numbness, loss of bladder or bowel control, fever, confusion, or breathlessness. These changes can signal a fracture, infection, spinal cord pressure, or another problem needing prompt assessment. If pain is steadily worsening or prevents sleep, eating, movement, or prescribed treatment, contact the oncology team promptly.

Questions for your doctor

Ask: Could this pain have a cause that bone-strengthening therapy does not treat? Do I need an examination or updated imaging? Which pain medicines are safe with my current treatment? What movement and weight-bearing are appropriate? Who should I call if the pain changes overnight?

Questions people ask

Should I stop bone-strengthening therapy if pain continues?

Do not stop or alter it without advice; ask the oncology team to review the pain and treatment together.

Can persistent bone pain have more than one cause?

Yes. Bone, joint, muscle, nerve, injury, and treatment-related causes may overlap, so a focused review is useful.
