What if eyelid surgery does not relieve under-eye bags?

The lower eyelid can look puffy because of fat, loose skin, muscle shape, fluid retention, tear-trough hollowing, pigmentation, or a combination of these features. Surgery may improve one component while another remains. Swelling can also distort the early result and settle gradually. A photograph taken soon after surgery may therefore not show the final appearance.

Persistent asymmetry, tightness, pulling, or a change in the eyelid position needs assessment rather than self-treatment. The surgeon can compare your current appearance with the preoperative plan and determine whether healing is still progressing or whether a different concern is responsible.

Keep your follow-up appointments and follow the surgeon’s instructions about wound care, activity, medicines, and eye protection. Do not massage the area, apply fillers, or arrange another procedure until the operating clinician has assessed it. Contact the surgical team urgently for sudden vision change, severe eye pain, rapidly increasing swelling, heavy bleeding, inability to close the eye, or marked redness with discharge. Sudden one-sided swelling or severe headache after surgery also warrants immediate assessment.

Ask: Is the appearance due to swelling or a remaining structural feature? Has the eyelid position healed as expected? Could hollowing or pigmentation be creating a shadow? When is it safe to consider any further treatment? Which symptoms require immediate contact?
