What if antibiotic treatment does not relieve coughing up phlegm?

Antibiotics act on certain bacterial infections, so they may not improve a cough caused by a virus, asthma, allergy, reflux, or irritation. Phlegm can also remain while inflamed airways recover, even when an infection is settling. A clinician may need to listen to your chest, review the colour and amount of mucus, and ask about breathlessness, wheezing, fever, smoking, and any medical conditions. Do not assume that persistent phlegm means the antibiotic has failed or that another antibiotic is needed.

Take the prescribed antibiotic exactly as directed unless the prescriber tells you to stop, and do not use leftover medicine or share it. Contact the prescriber if symptoms are not improving, are worsening, or return after seeming to settle. Drink fluids if you can, avoid smoke and other airway irritants, and rest. Ask before using a cough medicine, especially if you are pregnant, have another condition, or take regular medicines. A review may lead to an examination, a change in treatment, or advice for a non-infectious cause.

Seek urgent medical help for severe difficulty breathing, blue or grey lips, chest pain, confusion, coughing blood, or a sudden marked deterioration. Prompt assessment is also sensible when fever persists with worsening breathing, you feel unusually weak, or you have significant lung disease or a weakened immune system. A clinician can judge urgency from the whole pattern, not from phlegm colour alone.

Ask: What causes could explain this cough? Do I need a chest examination or further testing? Should I continue this antibiotic, and what changes should I report? Which treatments are suitable for clearing mucus? Could my inhalers, reflux, allergies, or smoking be contributing?
