What If Abscess Drainage Does Not Relieve Painful Skin?

If abscess drainage does not relieve painful skin, contact the treating clinician because the wound may need reassessment or the pain may have another cause. Some soreness can remain while the area heals, but pain that is worsening, severe, or not settling deserves a clinical review.

After a pocket of pus is opened and drained, surrounding tissue may still be inflamed. The opening can also close before all the material has left, allowing pressure and tenderness to return. Pain may come from irritation around the wound, an infection that is spreading beyond the original area, or a separate skin problem. A clinician may check the wound, nearby skin, and any dressing or packing, then decide whether it needs cleaning, another procedure, a swab, or prescription treatment. Do not assume that ongoing pain alone identifies the cause.

Keep the area clean and follow the aftercare instructions you were given. Wash your hands before touching the dressing, and do not squeeze, cut, or probe the wound. Use pain relief only as directed on the label or by your clinician, taking account of conditions or medicines that may make a product unsuitable. If packing or a dressing was placed, ask the treating service when it should be changed or removed rather than doing this earlier yourself. Arrange a review if the pain is not improving, returns after initially easing, or makes normal movement difficult.

Seek urgent medical help if redness or swelling is spreading, the skin becomes increasingly hot, pus or bleeding is increasing, or you develop fever, chills, marked weakness, confusion, or rapidly worsening pain. Prompt assessment is also needed if the abscess is near the eye, mouth, genitals, or spine, or if you have diabetes, a weakened immune system, or difficulty fighting infections. Call emergency services for trouble breathing, fainting, or severe confusion.

Ask: Does the wound look as though it has drained fully? Could the pain be coming from nearby inflammation or another skin condition? Do I need a wound check, a repeat drainage procedure, a swab, or different treatment? Which pain medicines are appropriate for me, and what changes should prompt urgent help?
