What happens if shoulder instability is not treated?

Short answer
If shoulder instability is not treated, the shoulder may continue to slip or dislocate, causing pain, weakness and reduced confidence with arm movement.
This answers one question. It is not a diagnosis and does not replace an assessment by a licensed doctor.
At a glance
- Common experience
- Slipping, giving way or apprehension during certain arm positions
- Emergency sign
- A shoulder that is out of place with numbness or a cold, pale hand
How instability can progress
Shoulder instability means the ball of the shoulder joint does not stay centred reliably. A person may feel slipping, giving way, popping or apprehension when the arm is raised or rotated. Repeated episodes can stretch or injure stabilising tissues and may make ordinary movements feel unsafe. Shoulder weakness can develop from pain, guarding or tissue injury. Recurrent dislocation can also affect nearby bone, cartilage or nerves, while shoulder osteoarthritis may develop later in some people. Popping and clicking alone do not prove instability.
What to do next
Book an orthopaedic or physiotherapy assessment after a first slipping episode, a dislocation or repeated feelings of giving way. Assessment considers how the episode happened, joint laxity, strength, control and any associated injury. Physiotherapy may build rotator cuff and shoulder-blade control, then progress to task-specific stability. Shoulder Stabilisation Surgery may be discussed when instability keeps recurring, a structural injury is present or rehabilitation has not restored reliable function. Avoid positions that repeatedly provoke slipping until reviewed.
When to seek urgent care
Go to urgent care if the shoulder is visibly out of place, cannot be moved, or has severe pain after an injury. Numbness, tingling, a cold or pale hand, or sudden loss of pulse or strength also needs immediate assessment. Do not try to force a dislocated shoulder back into position yourself.
Questions for your doctor
Is the joint unstable, and which structures are involved? Do I need imaging after the episode? Which positions should I avoid? How will rehabilitation progress, and what findings would support stabilisation surgery?

Questions people ask
Can untreated instability lead to dislocation?
Yes. An unstable shoulder may have repeated subluxations or complete dislocations, particularly in positions that stress the joint.
Can physiotherapy help shoulder instability?
It can improve strength, coordination and control for many people, although the plan depends on the injury history and joint structure.
