What happens if shoulder impingement is not treated?

Short answer
If shoulder impingement is not treated, pain and restricted movement may continue, making overhead tasks harder and potentially causing weakness from reduced use.
This answers one question. It is not a diagnosis and does not replace an assessment by a licensed doctor.
At a glance

- Overhead reaching, dressing, lifting and sleeping on the shoulder

- Assessment, activity modification and graded rehabilitation
What persistent symptoms can mean
Shoulder impingement occurs when tissues around the shoulder become irritated during certain movements, often when the arm is lifted or reaches overhead. Continuing painful activity can keep the area sensitive and encourage guarded movement. Over time, reduced use may affect shoulder strength and range. Pain when lifting the arm and shoulder pain can also arise from a rotator cuff problem, frozen shoulder, arthritis or another condition, so the label should be confirmed by examination.
Steps that may help
Arrange an assessment if symptoms persist, disturb sleep or interfere with work, dressing or sport. A clinician may examine movement and strength and recommend a graded rehabilitation plan. Physiotherapy can address shoulder control, posture, strength and the loads that provoke symptoms. A Cortisone Injection may be discussed for selected short-term symptom relief. Subacromial Decompression is reserved for particular cases after assessment and appropriate non-surgical care; it is not automatically needed for every painful shoulder.
When to seek care
Seek prompt care after a fall or sudden injury, or if the shoulder becomes visibly deformed, hot, red or swollen. Obtain urgent assessment for sudden inability to lift the arm, marked new weakness, chest pain, breathlessness, fever or unexplained severe pain. Persistent night pain or steadily worsening movement also warrants review.
Questions for your doctor
Which shoulder structures are involved? What movements should I modify? How long should I try rehabilitation before reviewing progress? Would an injection help my goals, and what findings would make surgery reasonable?

Questions people ask
Can shoulder impingement cause weakness?
Pain can inhibit muscle use, and prolonged guarding may reduce strength; new marked weakness needs assessment for other causes.
Is surgery always required?
No. Many care plans begin with activity changes and rehabilitation, while surgery is considered only in selected circumstances.
