What happens if scabies is not treated?

Scabies mites live in the outer layer of skin and trigger an itchy immune reaction. New bumps, burrows, or a spreading [Skin Rash](/symptoms/skin-rash/) may appear while the infestation continues.

Repeated scratching can cause open sores, crusting, and a secondary bacterial infection. Scabies can also pass through prolonged close skin contact and may circulate among household members or intimate contacts if only one person is treated. People with weakened immunity may develop a much heavier crusted form and need prompt medical care.

Wash clothing, towels, and bedding as directed, and place items that cannot be washed aside according to local clinical advice. Avoid sharing bedding or close physical contact until treatment instructions say the risk has been addressed. Itching can continue for a while after successful treatment; ongoing new burrows or new lesions need review.

Seek prompt care for pus, spreading warmth, increasing pain, fever, extensive crusting, or rapidly worsening sores. Get urgent help if a person with reduced immunity has a widespread crusted rash or feels unwell. Ask for assessment when infants, older adults, pregnant people, or several household members may be affected.
