After an epidural steroid injection, you may have temporary soreness, numbness, or a short-lived increase in pain before the steroid's effect develops over the following days. Your pain may improve quickly and then return as that numbing medicine wears off; this does not by itself show whether the steroid will help. Mild tenderness at the injection site is common after the needle has been removed. Some people notice flushing, trouble sleeping, or a temporary change in blood glucose after a steroid medicine; people with diabetes should follow the monitoring plan given by their clinician. A brief flare of back or leg pain can occur. Relief, when it occurs, may be gradual rather than immediate. The injection reduces inflammation around irritated spinal nerves, but it does not remove a disc bulge or repair structural pressure.

Note changes in pain, sensation, strength, and walking so you can discuss the response at follow-up.

Contact the clinic promptly for fever, increasing redness or drainage at the injection site, severe or worsening pain, or a severe headache that is worse when upright. Seek emergency care for new or worsening leg weakness, loss of bladder or bowel control, numbness around the genitals or inner thighs, fainting, trouble breathing, or signs of a serious allergic reaction. These symptoms need assessment because they can signal infection, bleeding, nerve compression, or another complication rather than a routine post-injection response.

Ask when you should expect an effect, which activities are suitable for you, and how the response will be reviewed. Clarify what to do if your pain returns after the numbing medicine wears off, whether you should continue physiotherapy, and which symptoms require a same-day call. If the injection does not provide enough relief, ask what other options match the cause of your symptoms; a procedure such as microdiscectomy may be considered only when the clinical picture supports it.
