What does testicular lump feel like with germ cell tumour?

A germ cell tumour may feel like a firm lump within the testicle, or like the testicle has become heavier, larger, or less even than before. Some people notice a dull ache in the groin or scrotum, while others have no pain. The change may be easier to notice during washing or dressing than during a brief glance.

Several non-cancerous problems can also cause a lump or swelling, including fluid, inflammation, or a change beside the testicle. Feeling the area cannot identify the cause. A clinician may examine both testicles and arrange an ultrasound to distinguish the testicle from nearby structures.

Arrange a prompt appointment if you find a new lump, persistent enlargement, or a change in firmness. Note when it began, whether it is changing, and whether you have pain, fever, urinary symptoms, or a recent injury. Do not repeatedly press the area to test it, because this can increase soreness without clarifying the cause.

At assessment, tell the clinician about medicines and previous scrotal problems. An ultrasound uses sound waves and does not use radiation; further tests are decided from the examination and imaging rather than from touch alone.

Seek emergency care for sudden severe testicular pain, rapid swelling, nausea or vomiting with pain, or a testicle that appears higher or differently positioned. These features can signal a time-sensitive problem. A lump without pain still needs prompt assessment, even when there is no urgent pain.
