What do swollen glands feel like with scarlet fever?

The lymph nodes in the front or sides of the neck may become enlarged and sore. They can feel like several small lumps beneath the skin and may hurt when you swallow or turn your head. Scarlet fever often occurs with a sudden sore throat, fever, a red tongue, and a fine rough rash that can feel like sandpaper. The rash may be more noticeable in skin folds. Other throat infections can cause similar nodes, so the swelling does not by itself confirm scarlet fever.

Arrange medical advice when a sore throat, fever, neck swelling, and rough rash occur together. Until assessed, avoid sharing cups, cutlery, or toothbrushes and wash hands after coughing or blowing the nose. Encourage fluids and soft foods if swallowing hurts. Use age-appropriate fever or pain relief only as directed. If treatment is prescribed, take it exactly as instructed and ask when the person can return to close-contact settings.

Seek prompt assessment because scarlet fever is a bacterial illness that may need specific treatment. Get urgent help if there is drooling, noisy or difficult breathing, inability to swallow fluids, severe dehydration, marked sleepiness, or rapidly worsening throat pain. Have a clinician review nodes that become red, hot, very painful, or continue enlarging after the throat illness improves.
