What does skin ulcer feel like with pyoderma gangrenosum?

The ulcer may begin as a small inflamed bump, blister, or pustule and then become an open sore with a ragged edge. The surrounding skin can look dusky, purple, blue, or red and may feel swollen or intensely sensitive. Pain can seem greater than the wound looks at first. Pyoderma Gangrenosum is an inflammatory condition, and minor injury or vigorous manipulation may worsen some lesions, so the diagnosis requires careful clinical assessment rather than a home test.

Cover the area with a non-stick dressing if advised, and handle it gently during washing and dressing changes. Avoid squeezing, scraping, or attempting to remove the ulcer edge. Keep a record of new pain, drainage, fever, or the speed of change, and tell the clinician about inflammatory bowel disease, arthritis, blood disorders, or a recent procedure. Treatment plans are individualized and may require coordinated dermatology care.

Contact a clinician promptly if a painful ulcer is enlarging, developing a dark or purple edge, or appearing after a minor injury or procedure. Seek urgent care for uncontrolled bleeding, fever, confusion, severe weakness, spreading redness, or rapidly increasing pain. A very painful open wound should not be self-treated as a routine infection because the wrong procedure can aggravate inflammation.
