Short answer

Palpitations is a common presentation of Iron Deficiency Anaemia. You may feel a rapid, forceful or fluttering heartbeat, sometimes after walking or climbing stairs and sometimes while resting. Tiredness, breathlessness on exertion, dizziness, headaches, pale skin or reduced stamina can occur at the same time. The sensation is not specific, so an assessment is needed rather than relying on how it feels.

This answers one question. It is not a diagnosis and does not replace an assessment by a licensed doctor.

Why iron deficiency can affect heartbeat

Iron deficiency anaemia reduces the blood’s ability to carry oxygen. The heart may compensate by beating faster or more forcefully to deliver oxygen around the body, which can make the heartbeat noticeable. Blood loss, low iron intake, reduced absorption and other conditions can contribute. Blood tests help confirm anaemia and investigate the underlying reason for it.

Arrange an assessment

Book a medical review for new or recurrent palpitations with fatigue, breathlessness or dizziness. Mention heavy or prolonged periods, bleeding, stomach symptoms, dietary restrictions, pregnancy, recent illness and medicines that may affect bleeding. Do not take high-dose iron without advice; the correct treatment depends on test results and the cause of the deficiency. Keep a note of triggers and episode duration.

When to seek urgent care

Get emergency help for palpitations with chest pain, severe breathlessness, fainting, collapse, confusion or sudden marked weakness. Seek prompt assessment for heavy ongoing bleeding, black stools, vomiting blood, or palpitations that remain fast and make you feel unwell. Sit down and avoid driving if you feel faint.

Questions for your appointment

Ask which blood tests are needed, whether the cause of iron loss needs investigation, and how soon results should be reviewed. Discuss treatment options, side effects and food or medicine interactions. Tell the clinician whether symptoms change with activity and whether anyone in your family has a blood disorder or rhythm problem.

Questions people ask

Can iron deficiency anaemia cause a pounding heart?

Should palpitations with fatigue be investigated?

Yes. A clinician can check blood counts and iron status while looking for the reason for the deficiency and considering other causes.
