What does pain during sex feel like with endometriosis?

People may describe a deep ache, pressure, cramping, or a sharper pain when the cervix or pelvic structures are moved. The discomfort can be stronger in certain positions or at particular points in the menstrual cycle. Some people feel soreness for minutes or hours after sex. Surface burning or dryness can occur too, but deep pain is the pattern often associated with pelvic endometriosis. Pain can vary from one encounter to another.

This symptom can have causes besides endometriosis, so the sensation alone cannot identify its cause. A clinician considers its timing, location, bleeding, bowel or bladder symptoms, and examination findings.

Pause or change activity when pain begins; continuing through it can make the experience harder and may increase pelvic muscle tightening. More time for arousal, a water-based lubricant, shallower penetration, and positions you control may reduce pressure. Note when the pain occurs, where you feel it, and whether it relates to your cycle. Arrange an assessment if pain is recurring, affects intimacy, or comes with pelvic pain outside sex. Treatment may address endometriosis, pelvic muscle tension, or another contributor.

Seek prompt medical care for severe or rapidly worsening pelvic pain, faintness, fever, vomiting, heavy bleeding, or a positive pregnancy test with pain or bleeding. Contact a clinician soon if sex causes repeated pain, new bleeding, unusual discharge, painful urination, or persistent pain afterward. These features need assessment rather than being attributed to endometriosis without an examination.
