A clinician may consider laser toning for uneven brown or grey-brown pigmentation, including some cases of melasma, dark underarms, dark inner thighs and other localised darkening. It may also be discussed for selected blue-grey pigmentation such as nevus of Ota. The response depends on the depth and cause of pigment, your skin type and whether an ongoing trigger is present.

Laser toning uses a Q-switched laser at settings intended to affect pigment while limiting heat exposure. It is not a universal treatment for every dark mark, and melasma can return or become more noticeable after irritation or light exposure. Darkening related to friction, inflammation, medication or a condition such as acanthosis nigricans may need its underlying driver addressed alongside any procedure.

Planning treatment safely

Start with an examination to identify whether the colour change is melanin, a vascular change, a mole or another skin problem. Tell the clinician about recent tanning, pregnancy, medicines, previous pigment treatments, cold sores and a tendency to develop post-inflammatory darkening. Use the recommended broad-spectrum sun protection and avoid picking, scrubbing or adding unapproved bleaching products.

The clinician may recommend a test spot or a staged plan, particularly for richly pigmented or sensitive skin. Several appointments can be needed, and maintenance or treatment of the trigger may matter more than repeated laser sessions. Alternatives can include topical therapy, a different laser platform or an approach such as picosecond laser treatment, depending on the diagnosis.

When to seek medical care

Tell the clinic about severe pain, blistering, crusting, spreading redness, pus or a rapidly changing light or dark patch after treatment. Seek assessment for a new, bleeding, asymmetrical or rapidly changing pigmented spot rather than treating it cosmetically first. Urgent care is appropriate for facial swelling with breathing difficulty or other signs of a severe reaction.
