What does knee pain feel like with ACL tear?

Knee Pain is a common presentation of ACL Tear. Pain may be sharp at the moment of injury, then become aching or tight as swelling develops. The knee may feel unstable when turning, stepping down, or changing pace. Some people hear or feel a pop, but that detail is not present in every injury and cannot confirm the diagnosis.

Pain can also arise from injured meniscus, bone, cartilage, or other ligaments. An examination is needed to understand which structures are involved and whether the knee is safe to load.

Stop the activity and protect the knee from twisting. Use a comfortable walking aid if weight-bearing is difficult, and raise the leg while resting. Cold therapy wrapped in a cloth may help comfort for short periods. Avoid returning to sport or testing pivoting until a clinician has assessed the knee. Arrange an orthopaedic or physiotherapy review, especially if swelling or instability persists.

Seek urgent assessment if the knee is visibly deformed, remains locked, becomes rapidly swollen, or you cannot bear weight. A numb or cold foot, severe worsening pain, or an open injury requires urgent care. Fever with a hot, red joint also needs prompt medical review.

Ask which structures may be injured, whether imaging is appropriate, and how much weight-bearing is safe. Discuss a rehabilitation plan, return-to-activity milestones, and how to reduce the risk of another giving-way episode.
