What does joint pain feel like with gout?

Gout pain may feel sharp, burning or pulsating. The joint can become so sensitive that light contact from clothing or bedding is uncomfortable. The skin over it may look red and feel warmer than nearby skin. An attack often starts in a single joint, commonly the base of the big toe, although the ankle, knee, wrist or another joint can be involved. Pain may build quickly and make walking or using the joint difficult.

These features describe a pattern, not a diagnosis. Other causes of a hot, swollen joint can need prompt treatment, so assessment matters, especially with a first episode.

Rest the affected joint, raise it when practical and use a wrapped cold pack for short periods. Avoid pressure on the area and drink fluids unless a clinician has told you to restrict them. A pharmacist or clinician can advise on suitable pain relief, taking account of kidney disease, stomach ulcers, blood thinners and other medicines. Do not start or change gout medicines without guidance.

Arrange medical advice promptly for a first sudden attack, a joint that is hot and swollen, or pain that is not settling. Seek urgent care if fever, chills, feeling very unwell, rapidly spreading redness or an inability to move the joint accompanies the pain, because infection can resemble gout.

Ask whether the joint needs examination or fluid testing, which pain relief is appropriate with your health history, and how to prevent further attacks if gout is confirmed. Mention when the pain began, which joint changed, any fever, and every medicine or supplement you take.
