What does delayed puberty feel like with imperforate hymen?

An imperforate hymen is a membrane that completely covers the vaginal opening. Puberty changes such as breast development or pubic hair may still occur, while menstrual blood cannot leave the vagina normally. This can lead to no visible periods, a feeling of fullness low in the abdomen, back discomfort, or cramps that return in a monthly pattern. Some people notice difficulty passing urine or constipation if collected blood presses on nearby structures.

Arrange an appointment with a gynaecologist or adolescent health clinician if periods have not started alongside other puberty changes. The assessment may include questions about symptoms, an external examination, and imaging such as an ultrasound. If an imperforate hymen is confirmed, a small procedure can create an opening so fluid can drain and future periods can pass. Do not try to open the tissue yourself or insert objects to test the opening.

Seek urgent medical help for severe or worsening lower-abdominal pain, vomiting, inability to pass urine, fainting, fever, or a rapidly increasing abdominal swelling. These symptoms can signal pressure or another problem that needs timely examination. New pelvic pain without these warning signs still deserves a planned clinical review.
