Chest sensations during panic
A panic episode can activate the body's alarm response, tightening muscles and changing breathing. Chest pressure may appear with a racing or pounding heartbeat, trembling, sweating, dizziness, tingling, nausea, a choking sensation, or fear of losing control. The intensity can be frightening and may build quickly before easing. Panic disorder involves recurrent unexpected panic episodes and concern or behaviour changes related to them; one episode alone does not establish that diagnosis. Similar symptoms can occur with heart or lung problems.
What you can do during an episode
If you can do so safely, sit down, place both feet on the floor, and focus on a slow, comfortable breathing rhythm rather than repeated deep breaths. Remind yourself that the sensation is a body alarm while still checking for warning signs. Once settled, record what happened and arrange a clinical review if episodes recur, cause avoidance, or interfere with sleep, work, or daily activities. A clinician can discuss psychological treatment and other options.
When to get urgent help
Call emergency services for severe or unfamiliar chest pressure, fainting, serious breathing difficulty, blue or grey lips, confusion, or pain spreading to the arm, jaw, back, or neck. Seek urgent assessment if symptoms continue, follow exertion, or do not resemble your previous episodes. Panic can cause chest tightness, but it should not be used to explain away a potentially serious new symptom.
