What the bleeding may be like

Diverticular Disease involves small pouches in the bowel wall. If a blood vessel near one of these pouches bleeds, the episode can begin suddenly and may not cause pain. Blood may be bright red, deep red, or mixed with stool. Diverticular inflammation is a different situation: it can cause steady lower abdominal pain, fever, tenderness, constipation, or diarrhoea, and bleeding is not always the main feature.

Blood in stool is a common presentation of Diverticular Disease, but the symptom does not establish that diagnosis. Haemorrhoids, fissures, polyps, inflammatory bowel disease, and other conditions can also cause rectal bleeding. The amount, colour, associated symptoms, medicines, and examination findings help guide assessment.

What to do after noticing blood

Arrange medical advice for any new or unexplained bleeding, even if it stops. Note when it began, whether it was mixed with stool, and whether you had pain, fever, dizziness, or a change in bowel habits. Mention aspirin, anticoagulants, or other medicines that affect clotting. Do not take anti-inflammatory painkillers or alter prescribed medicines without checking with a clinician.

When it needs urgent attention

Seek emergency help for heavy or repeated bleeding, clots, faintness, dizziness, rapid weakness, pale or clammy skin, severe abdominal pain, fever, vomiting, or black tar-like stool. Urgent review is also needed when bleeding continues or you cannot keep fluids down. Significant bowel bleeding can worsen quickly even when it starts without pain.
