What do bladder or bowel changes with back pain feel like with cauda equina syndrome?

Bladder symptoms may include reduced awareness that the bladder is full, difficulty starting the urine stream, needing to strain, incomplete emptying, or new leakage. Bowel changes can include reduced awareness of needing to open the bowels, difficulty controlling stool, or altered sensation during wiping. Numbness around the inner thighs, buttocks, genitals, or saddle area and weakness in one or both legs can occur alongside back pain. These symptoms reflect possible pressure on nerves that control the bladder, bowel, and lower limbs. Other causes exist, but the combination must not be assessed casually at home.

If these changes are new or worsening, stop strenuous activity and seek emergency medical assessment now. Do not drive yourself if your legs are weak or sensation is impaired. Tell the emergency team when the back pain, urinary or bowel change, numbness, and weakness began. Bring a list of medicines and mention recent injury, spinal treatment, or fever if present.

Go to an emergency department immediately for new difficulty passing urine, loss of bladder or bowel control, numbness in the saddle area, or progressive leg weakness with back pain. Fever, severe unrelenting pain, or symptoms after significant injury also require urgent evaluation. Emergency imaging and specialist assessment may be needed to identify nerve compression.
