What does abdominal swelling feel like with myelofibrosis?

Myelofibrosis can affect the spleen, an organ on the upper left side of the abdomen. When the spleen becomes enlarged, you may feel heaviness, pressure, tenderness, or a firm fullness beneath the left ribs. A large spleen can make you feel full after eating only a small amount because it presses on nearby organs. The abdomen may look more prominent, and bending or lying in certain positions may feel uncomfortable. Swelling can also have other causes, including digestive problems or fluid, so this symptom does not establish a diagnosis by itself.

Tell your haematology team about new or worsening fullness, pain, appetite changes, weight change, fever, night sweats, or altered bowel habits. Note where the pressure is located and whether meals or movement make it worse. Choose smaller meals if early fullness is affecting nutrition, and avoid activities that cause pain until you have guidance. Do not press repeatedly on a tender abdomen. Your clinician may check the spleen and blood results and may arrange imaging or adjust treatment if needed.

Get urgent help for sudden severe pain under the ribs, a rigid or rapidly enlarging abdomen, fainting, dizziness, unusual weakness, heavy bleeding, black stools, fever with chills, or trouble breathing. Contact your care team promptly if you cannot eat or drink because of pressure or if pain is increasing. Sudden abdominal symptoms need assessment rather than home observation.

Ask whether the spleen could be contributing to the swelling, how it will be monitored, and which symptoms suggest a complication. Ask what foods or activities may be easier, whether your current medicines can affect the sensation, and when you should contact the haematology team.
