What does abdominal pain feel like with ulcerative colitis?

Ulcerative colitis affects the lining of the large bowel, so discomfort is often felt low in the abdomen or across the lower abdomen. Cramps may build before a bowel movement and occur with urgency, frequent loose stools, mucus, or blood. During a flare, tiredness, reduced appetite, and a raised temperature can also occur. Symptoms vary, and abdominal pain by itself cannot establish the diagnosis.

Contact your gastroenterology team or primary-care clinician if cramps, urgency, or bleeding are new or worsening. Continue prescribed treatment unless your clinician gives different instructions, and drink fluids regularly when stools are loose. Ask before using anti-diarrhoeal medicines, especially if you have fever, bleeding, or significant abdominal swelling.

Seek urgent medical help for severe or escalating pain, a swollen abdomen, repeated vomiting, heavy or ongoing rectal bleeding, faintness, confusion, or inability to keep fluids down. A high temperature with worsening bowel symptoms also needs prompt assessment.
