What causes shoulder weakness?

Short answer
Shoulder weakness can result from a rotator cuff tear, pain that inhibits muscle use, instability, reduced conditioning, or a nerve problem from the neck.
This answers one question. It is not a diagnosis and does not replace an assessment by a licensed doctor.
At a glance
- Possible sources
- Rotator cuff, instability, pain inhibition, conditioning, or neck nerves
- Important clues
- Sudden onset, injury, arm control, tingling, and numbness
Different types of weakness
Some people feel weak because pain makes the muscles switch off or because movement has been avoided. True loss of strength can occur when a rotator cuff tendon is injured, the shoulder feels unstable, or a nerve is compressed. Weakness from the neck may accompany neck pain, tingling, numbness, or symptoms traveling into the arm. Difficulty lifting the arm, reaching a shelf, or controlling it while lowering can provide useful clues. A shoulder that repeatedly slips may lose confidence during certain positions. The symptom does not identify one diagnosis by itself; timing, injury history, sensation, and examination matter.
What to do next
Stop heavy or overhead tasks that cause the arm to give way, but keep comfortable movement so the shoulder does not become more restricted. Do not repeatedly test strength against pain. Arrange assessment if weakness is new, persistent, worsening, or followed an injury. A clinician can compare both sides, assess the neck and nerves, and decide whether imaging or rehabilitation is appropriate. Physiotherapy may address control and strength; a confirmed major tendon injury may lead to discussion of Rotator Cuff Repair. Related reading includes Rotator Cuff Tear and Shoulder Instability.
When to seek care
Seek urgent help for sudden weakness after trauma, a deformed shoulder, inability to lift the arm, or weakness with facial drooping, speech trouble, or difficulty walking. Prompt assessment is also needed for new numbness, a cold or discoloured hand, severe swelling, or rapidly worsening symptoms. Book a routine review when weakness affects work, dressing, sleep, or sport.
Questions for your doctor
Is my weakness pain-related or due to a tendon, joint, or nerve? Should my neck be assessed as well? Which movements are safe while I wait? Would physiotherapy, imaging, or Rotator Cuff Repair be relevant?

Questions people ask
Can shoulder pain make the arm feel weak?
Yes. Pain can reduce muscle activation and make lifting feel weak even when the tendon and nerve are intact.
When is shoulder weakness urgent?
Sudden weakness after injury, weakness with neurological symptoms, or a cold or discoloured hand needs urgent medical assessment.
