What Causes Post-Inflammatory Erythema?

Short answer
Post-inflammatory erythema develops when inflammation, often from acne, leaves lingering redness as the skin and tiny blood vessels heal.
This answers one question. It is not a diagnosis and does not replace an assessment by a licensed doctor.

Why a red mark remains
Inflammation widens small blood vessels and signals immune activity around an irritated follicle or injured patch of skin. After the original spot settles, the vessels and surrounding tissue may take time to return to their usual appearance. The mark can look pink, red or purplish, particularly in areas where the skin is easily flushed. It is different from brown or grey post-inflammatory hyperpigmentation, although both can follow the same acne lesion.
The intensity and duration depend on the depth of inflammation, repeated irritation, picking and individual healing characteristics. New breakouts can create additional marks, making the redness seem to spread even when each mark is healing separately.
Care while it fades
Treat active acne gently and avoid picking, squeezing, harsh scrubs and unnecessary friction. Use a broad-spectrum sunscreen because ultraviolet exposure can make lingering discoloration more noticeable. A bland moisturizer can reduce barrier irritation. If redness persists or is distressing, a dermatologist can assess whether prescription treatment or a vascular-focused procedure is suitable; skin tone, sensitivity and the cause of the original inflammation guide that choice.
When to arrange a review
Seek an assessment if redness is painful, warm, rapidly expanding, associated with pus or accompanied by fever, since those features can indicate active infection or inflammation rather than a fading mark. A review is also useful when marks keep appearing, the original acne is uncontrolled, or the color is changing in a way that is difficult to distinguish from pigmentation.
Questions for your doctor
Ask whether the mark is erythema or pigmentation, how to control the acne causing new lesions, and which treatment is safest for your skin tone and sensitivity.

Questions people ask
Is post-inflammatory erythema the same as a scar?
It is usually a color change after inflammation rather than a permanent indentation or raised scar, although both can occur after acne.
Can picking make the redness last longer?
Yes. Picking can deepen inflammation and injure nearby tissue, which may prolong redness and create further marks.
