What causes persistent abdominal pain?

Short answer
Persistent abdominal pain has many possible causes, including digestive illness, inflammation, organ problems, and sometimes cancer.
This answers one question. It is not a diagnosis and does not replace an assessment by a licensed doctor.
At a glance
- Many causes
- Digestive, urinary, reproductive, inflammatory, and other conditions can cause ongoing pain.
- Track details
- Location, timing, triggers, and associated changes help guide assessment.
What the pattern can tell a clinician
The location, timing, severity, and triggers help narrow the possibilities. Pain after meals may involve the stomach, gallbladder, pancreas, or bowel. Cramping with altered bowel habits can reflect intestinal causes, while pelvic pain may involve reproductive or urinary organs. Neuroendocrine tumours, ovarian cancer, pancreatic cancer, small bowel cancer, and stomach cancer can present with abdominal pain, but pain alone does not establish any of these conditions. The same symptom can also come from infection, constipation, ulcers, stones, or muscle strain.
How to prepare for assessment
Keep a short record of where the pain occurs, when it starts, what makes it better or worse, and any changes in appetite, weight, bowel movements, urine, or menstrual bleeding. A clinician may examine your abdomen and choose blood tests, imaging, or other tests based on the pattern. Do not start restrictive diets or pain medicines without checking whether they are suitable for you.
When pain needs prompt attention
Arrange medical review when pain persists, recurs, or interferes with eating, sleep, work, or daily activity. Seek urgent help for severe or rapidly worsening pain, a rigid or swollen abdomen, repeated vomiting, blood in vomit or stool, fainting, fever, jaundice, or inability to pass stool or urine. These warning signs require assessment regardless of the eventual cause.
Questions for your doctor
Ask which causes best fit the pain’s location and pattern, what examination or tests are needed, and which changes should trigger urgent care. If cancer is suspected or confirmed, ask about pain control and supportive care alongside investigations and treatment.
Questions people ask
Does persistent abdominal pain mean cancer?
No. Many non-cancerous conditions cause ongoing pain. Cancer is one possibility among several and cannot be diagnosed from pain alone.
When is abdominal pain an emergency?
Severe worsening pain, fainting, repeated vomiting, bleeding, fever, jaundice, or inability to pass stool or urine needs urgent assessment.
