What causes periorbital hyperpigmentation?

Short answer
Periorbital hyperpigmentation is caused by a mix of inherited pigmentation, sun exposure, irritation, visible vessels, and shadows from the eye-area anatomy.
This answers one question. It is not a diagnosis and does not replace an assessment by a licensed doctor.

What causes periorbital hyperpigmentation?
Periorbital hyperpigmentation is caused by a mix of inherited pigmentation, sun exposure, irritation, visible vessels, and shadows from the eye-area anatomy. More than one factor can be present at the same time, so the colour may look brown, grey, blue, or purplish rather than having one fixed appearance.
What the dark appearance can mean
Extra melanin can make the skin look brown, especially after repeated sun exposure or inflammation. Rubbing the eyes, eczema, contact irritation, or allergies may inflame delicate skin; the area can remain darker after the irritation settles. Atopic dermatitis can affect the eyelid area and contribute to rubbing and post-inflammatory darkening; see the page on Atopic Dermatitis for related information.
Some dark circles are mainly optical. Thin eyelid skin may show underlying blood vessels, while hollows, puffiness, or a deep tear trough can cast a shadow. Dehydration, tiredness, and nasal congestion may make the area look more noticeable without being the original cause of pigment.
What you can do
Begin by treating the skin gently: avoid rubbing, use fragrance-free products around the eyes, and stop any product that stings or causes scaling. Use broad-spectrum sun protection and sunglasses because ultraviolet exposure can deepen pigment. If itching, redness, or flaking is present, a clinician can identify the trigger and recommend an eye-area treatment that is suitable for thin eyelid skin.
A dermatologist can distinguish pigment from shadows or visible vessels during an examination. Treatment depends on that distinction and may include trigger control, prescription medication, or a carefully selected procedure. Do not apply strong bleaching products or steroid creams near the eyes without medical guidance.
When to seek medical care
Arrange an assessment if the colour change is new, clearly affects only one side, spreads, or comes with persistent itch, rash, swelling, pain, or vision changes. Seek prompt care for marked swelling, severe eye pain, or a sudden change in vision. A clinician should also review darkening that continues despite stopping an irritating product, because the underlying skin condition may need treatment.
Questions for your doctor
Ask whether the appearance is mainly pigment, visible vessels, or shadowing; whether eczema, allergy, or another irritation is affecting the eyelids; which sunscreen and eye-area products are appropriate; and whether treatment could worsen sensitivity or pigmentation.

Questions people ask
Can rubbing my eyes make dark circles worse?
Yes. Repeated rubbing can irritate eyelid skin and leave darker colour after inflammation, especially when itching or eczema is present.
Are all dark circles caused by extra pigment?
No. Thin skin, visible blood vessels, puffiness, hollows, and shadows can create a dark appearance even when extra pigment is not the main factor.
When should new darkening around one eye be checked?
Arrange a medical assessment when darkening is new, one-sided, spreading, or accompanied by rash, swelling, pain, or vision changes.
