What Causes Pain That Wakes You at Night?

Short answer
Pain that wakes you at night can reflect inflammation, a shoulder injury, infection, or pain from bone or joint conditions.
This answers one question. It is not a diagnosis and does not replace an assessment by a licensed doctor.

What night pain can indicate
Night pain is a symptom rather than a diagnosis. Shoulder pain may worsen when you lie on the affected side or when a torn rotator cuff is compressed. Frozen Shoulder can cause aching and marked restriction of movement, often making sleep positions difficult. Infection in bone or a joint can cause persistent pain with warmth, swelling, fever, or feeling unwell. A benign bone tumour can also be associated with localized bone pain, although only an assessment can identify its cause.
Tell a clinician whether the pain is new, steadily worsening, present at rest, linked to movement, or accompanied by swelling, fever, weight change, or reduced function.
What to do tonight and next
Try a supported position: place a pillow under the painful arm or between the knees, and avoid lying directly on a tender shoulder. Keep a brief record of when pain starts, what wakes you, and any daytime symptoms. Do not repeatedly test a painful joint or push through sharp pain. Arrange a medical review when night waking continues, especially if ordinary measures do not help.
When to seek care
Seek urgent care for severe pain with fever, a hot or swollen joint, sudden inability to move a limb, a recent major injury, or rapidly worsening symptoms. Prompt assessment is also needed for unexplained persistent bone pain, new weakness or numbness, or pain with feeling generally unwell.
Questions for your doctor
Ask whether the location and timing suggest a shoulder, bone, or joint source; whether you need an examination or imaging; and which sleep positions and pain-relief measures are suitable for you.

Questions people ask
Is night pain always serious?
No. Several musculoskeletal problems disturb sleep, but persistent or unexplained night pain should be assessed rather than ignored.
Why does shoulder pain worsen in bed?
Lying on the shoulder or keeping it in one position can increase pressure and stiffness around irritated tissues.
