What causes myelofibrosis?

The altered cells release signals that encourage fibrous tissue to build up in the bone marrow. As the marrow becomes less able to make healthy red cells, white cells, and platelets, the spleen or liver may help make blood cells instead and become enlarged.

Myelofibrosis can arise on its own, called primary myelofibrosis, or develop after another blood disorder. Essential thrombocythaemia and polycythaemia vera are examples of conditions that can precede secondary myelofibrosis.

Diagnosis usually combines a blood count, examination for an enlarged spleen, molecular testing, and sometimes a bone-marrow biopsy. These tests help distinguish myelofibrosis from other causes of anaemia, low platelets, or an unusual blood count.

Bring earlier blood results and details of bleeding, bruising, fevers, night sweats, itching, bone discomfort, or fullness under the ribs. Do not stop prescribed medicine without discussing it with the treating team.

Contact a clinician promptly for persistent fatigue, unexplained weight change, night sweats, fevers, easy bruising, or a feeling of fullness after small meals. Seek urgent care for heavy bleeding, severe breathlessness, chest pain, fainting, or sudden neurological symptoms.
