What causes metabolic syndrome?

Short answer
Metabolic syndrome develops when several metabolic changes occur together, often driven by obesity, insulin resistance, inactivity, genetics, and ageing.
This answers one question. It is not a diagnosis and does not replace an assessment by a licensed doctor.
At a glance
- What is measured
- Blood pressure, glucose, waist size and blood fats may be assessed.
- Symptoms
- The cluster can be present without obvious symptoms.
How the pattern develops
Metabolic syndrome is a cluster rather than one single symptom. Extra body fat, especially around the waist, can make cells respond less effectively to insulin. The body then has more difficulty regulating blood glucose and may produce more insulin. This metabolic strain can occur alongside raised blood pressure and unhealthy blood-fat patterns. Obesity is a common driver, but body size alone does not diagnose the syndrome. Family history, age, diet, limited activity, sleep problems and some hormonal conditions can influence risk.
What to do next
Ask a clinician to check blood pressure, waist measurement, blood glucose and cholesterol or other blood fats when appropriate. Share your family history, eating pattern, activity, sleep and medicines. Gradual changes such as regular walking, more fibre-rich foods, less highly processed food, improved sleep and stopping smoking can support metabolic health. Treatment may also include medicine for blood pressure, glucose or cholesterol when clinically appropriate.
When to seek care
Arrange a routine assessment if you have several risk factors, a growing waist, high blood pressure, abnormal blood tests or a family history of diabetes. Seek urgent care for chest pressure, severe breathlessness, fainting, sudden weakness or confusion. Metabolic syndrome may have no obvious symptoms, so checking measurements matters even when you feel well.
Questions for your doctor
Which measurements should be checked for me? Could obesity, sleep, medicines or family history be contributing? What change should I start first, and when should the tests be repeated?

Questions people ask
Is metabolic syndrome caused only by obesity?
No. Obesity can contribute, but insulin resistance, activity, genetics, age and other factors also matter.
Can metabolic syndrome improve?
The underlying risk factors can often be improved through lifestyle measures and treatment matched to the individual findings.
