What causes hallux rigidus?

Short answer
Hallux rigidus can develop when the big-toe joint becomes irritated, damaged, or arthritic, sometimes after turf toe.
This answers one question. It is not a diagnosis and does not replace an assessment by a licensed doctor.
At a glance
- Joint involved
- The joint at the base of the big toe
- Typical difficulty
- Pain or stiffness during push-off when walking
How it develops
The big toe needs to bend upward as you walk. Repeated loading, a previous injury, or changes in the joint surface can lead to inflammation, reduced movement, and bony enlargement around the joint.
Turf toe is an injury involving forced upward bending of the big toe and may contribute to later stiffness. Some people notice pain when pushing off, swelling around the joint, or difficulty wearing shoes with a narrow or flexible front.
What may help
Choose shoes with a roomy toe box and a firm or stiff sole to reduce bending at the painful joint. Temporarily reduce activities that repeatedly load the toe, such as sprinting or jumping, and use gentle movement without forcing the joint.
A foot and ankle clinician can assess toe movement and walking mechanics. Treatment may include footwear changes, activity modification, exercises, or other options matched to the joint findings.
When to seek care
Book an assessment when stiffness or pain affects walking, shoe choice, work, or sport, or when symptoms follow an injury. Seek prompt care for marked redness and warmth with fever, sudden severe pain, or a toe that looks deformed after trauma.
Questions for your doctor
Ask whether the joint shows arthritis or an old injury, how much toe movement is safe, which shoe features may reduce loading, and when a return to sport is appropriate.

Questions people ask
Is hallux rigidus the same as a bunion?
No. Hallux rigidus mainly limits movement at the big-toe joint, while a bunion is a change in alignment at that joint and the toe.
Can an old toe injury cause hallux rigidus?
It can contribute. An injury such as turf toe may irritate the joint and leave it stiff or painful over time.
