What the plantar plate does

A tear can cause forefoot pain, swelling, tenderness under a toe joint or a toe that begins to drift or lift. Examination and sometimes imaging help determine the injury’s extent and whether the joint is stable.

The early aim is to stop repeated stretching of the injured tissue. Later care focuses on restoring comfortable loading without allowing the toe to keep drifting.

How treatment progresses

A clinician may recommend a stiff-soled shoe, a walking aid or a period of protected weight-bearing when push-off is painful. Taping or splinting can hold the toe in a supportive position. Custom orthotics may reduce pressure beneath the affected joint and make everyday walking more comfortable.

Once pain and swelling settle, rehabilitation can address calf and foot strength, balance and a gradual return to normal footwear. Avoid high-impact activity and narrow shoes until advised. Surgery may be discussed for a displaced, unstable or persistent tear that does not improve with protection and rehabilitation.

Arrange a foot assessment if forefoot pain persists, the toe is changing position, or walking becomes difficult. Seek prompt care after sudden swelling, a visible deformity, inability to bear weight, a wound, marked redness or a cold or numb toe.
