What are the treatment options for fungal acne?

Malassezia folliculitis involves yeast associated with hair follicles and often appears as small, similar-looking itchy bumps. It can resemble acne, heat rash or bacterial folliculitis, so diagnosis matters before using acne products. A clinician may examine the pattern, itch, skin products, sweating and previous treatments.

Topical antifungal treatment is applied directly to affected skin. A medicated shampoo may be used as a short-contact wash on areas such as the scalp, chest or back when instructed. Oral antifungal treatment may be considered when involvement is extensive, persistent or unsuitable for topical care; it requires a review of medicines, liver health and interactions.

Use the prescribed product for the full recommended course and avoid adding several new acne treatments at once. Shower after heavy sweating, change out of damp clothing, and choose light, non-greasy skin products if they do not irritate you. Do not share medicated products, and keep shampoo away from the eyes and broken skin.

Improvement may be gradual, and bumps can recur when heat, sweat, occlusive products or certain medicines continue to contribute. If the rash does not respond, reassessment is useful because ordinary acne or another follicle condition may need a different approach.

Seek an assessment if bumps are painful, draining, rapidly spreading, leaving scars or accompanied by fever. Contact a clinician promptly for a widespread rash, facial swelling, breathing difficulty or a suspected medicine reaction. If oral treatment is prescribed, report yellowing of the skin, dark urine, unusual fatigue or severe nausea.

Ask how the diagnosis was made, whether a topical product or medicated shampoo is suitable, how long to leave it on, and when oral treatment would be considered. Ask which cosmetics, oils or hair products to pause and how recurrence can be reduced.
