Short answer

Treatment for chronic myeloid leukaemia usually starts with targeted therapy, while stem cell transplant is considered for selected situations when disease control is difficult.

This answers one question. It is not a diagnosis and does not replace an assessment by a licensed doctor.

At a glance

Usual first approach
Targeted therapy
Response checks
Blood and molecular tests
Selected option
Allogeneic stem cell transplant

How CML treatment works

CML is driven by an abnormal change in blood-forming cells. Targeted therapy is designed to block the signal produced by that change. Many people take it as a long-term tablet, with blood tests and molecular monitoring showing how deeply the leukaemia is responding.

If the first targeted medicine does not work well enough, causes troublesome effects, or resistance develops, the team may discuss another targeted medicine. An allogeneic stem cell transplant replaces diseased blood-forming cells with cells from a donor. It can involve intensive treatment and serious risks, so it is reserved for circumstances where its potential benefit justifies those risks.

Managing treatment safely

Take targeted therapy at the prescribed time and ask what to do after a missed dose or vomiting. Tell the team about every medicine, supplement, and herbal product because interactions can alter treatment levels. Keep scheduled blood tests even when you feel well; they help detect response and side effects.

Ask whether the treatment should be changed before stopping it yourself. If transplant is raised, ask about donor matching, preparation, hospital recovery, infection precautions, and support at home.

When to seek care promptly

Contact your team promptly for fever, chills, unusual bruising or bleeding, worsening breathlessness, severe abdominal discomfort, or marked weakness. Seek emergency care for chest pain, fainting, severe breathing difficulty, or sudden neurological symptoms.

Questions for your doctor

What result will show that my targeted therapy is working? How often will molecular and blood tests be done? What side effects need a same-day call? If resistance occurs, which alternatives or transplant pathway would be considered?

Questions people ask

Can I stop targeted therapy when my blood test improves?

Do not stop it without specialist advice; the team uses ongoing monitoring to decide whether a change is safe.

Is stem cell transplant used for every person with CML?

No. It is considered for selected cases, such as disease that remains difficult to control or becomes resistant to available medicines.
