How the shoulder joint changes

Osteoarthritis can develop when the smooth surfaces of a shoulder joint become less able to tolerate load. Age-related tissue change is one factor, but a previous fracture, dislocation or operation can alter joint shape and movement. Shoulder instability may repeatedly stress the joint, while a long-standing rotator cuff tear can change how the upper arm moves within the socket.

Work or sport involving frequent overhead lifting, forceful pushing or repeated throwing can add mechanical demand, particularly when strength and recovery do not match the workload. Inflammatory joint disease and some structural differences may also contribute. A risk factor does not establish the diagnosis, and shoulder pain can come from tendons, the neck or other tissues.

Supporting shoulder function

Use gradual load progression and alternate demanding overhead tasks with recovery. Strengthen the rotator cuff, shoulder-blade and upper-back muscles with guidance if pain or prior instability is present. Keep the shoulder moving within a comfortable range rather than protecting it completely. A physiotherapist can review technique, posture, work setup and sport-specific demands, then adjust the plan as strength and control improve.

When to seek care

Arrange an assessment for ongoing pain, night discomfort, loss of range, grinding, or difficulty dressing and reaching. Seek urgent care after a fall or forceful event if the shoulder looks out of place, cannot be lifted, becomes suddenly weak, or is accompanied by severe swelling. Chest pressure, breathlessness or sweating with shoulder pain needs emergency attention.
