How risk develops

Cauda equina syndrome occurs when the bundle of nerves at the bottom of the spinal canal becomes severely compressed. A sudden or large [Herniated Disc](/conditions/herniated-disc/) is a very common cause. [Spinal Stenosis](/conditions/spinal-stenosis/) can leave less room for the nerves, particularly when other changes add pressure. Trauma, bleeding, infection, or a growth can also compress this area.

Back pain or sciatica alone does not establish cauda equina syndrome. The concern rises when nerve compression affects bladder, bowel, sexual sensation, or both legs.

What to do with risk factors

If you have a known disc problem or narrowing, follow the treatment plan and report any new change in leg strength or sensation promptly. Keep a clear record of when symptoms began and whether they are progressing. Avoid delaying assessment to see whether severe nerve symptoms settle on their own; early examination helps clinicians decide whether emergency imaging and specialist treatment are needed.

Treat these symptoms as an emergency

Go to an emergency department immediately for new difficulty starting urination, loss of bladder control, loss of bowel control, numbness around the genitals or anus, or rapidly worsening weakness in both legs. Severe back pain with these changes also needs emergency assessment. Do not drive yourself if weakness or numbness makes travel unsafe.
