What are the first signs of hives?

A patch may start as a small itchy bump, then become a pale, pink, or red welt. Individual welts may join together, fade in one area, and appear elsewhere. Hives can affect any skin surface and may be triggered by an infection, medicine, food, heat, pressure, or another exposure. The rash itself is often temporary, but repeated episodes can point to chronic hives.

Swelling deeper in the skin, especially around the lips, eyelids, tongue, hands, or feet, is called angioedema and may occur with hives. Scratching can intensify discomfort without explaining the underlying cause.

Take a photo when the welts are visible and note recent foods, medicines, infections, exercise, heat, pressure, or new products. Keep the skin cool, wear loose clothing, and avoid rubbing the affected areas. A clinician or pharmacist can advise whether an oral antihistamine is suitable for you; do not start or stop a medicine without checking if you have other conditions, take regular medicines, or are pregnant.

Arrange medical advice if episodes keep returning, last beyond a short period, leave bruising, or are accompanied by fever or joint symptoms. A clinician may look for a pattern rather than one isolated trigger.

Call emergency services immediately for hives with trouble breathing, wheezing, throat tightness, faintness, confusion, or swelling of the tongue or mouth. These symptoms can signal a severe allergic reaction and should not be watched at home. Rapidly increasing facial swelling also needs urgent assessment.
