What Are the First Signs of Psoriasis?

Patches may develop on the elbows, knees, scalp, lower back, or other areas. They can feel raised because skin cells build up more quickly than usual. Some people first notice persistent scaling or a spot that repeatedly becomes irritated; others notice small pits or roughness in the nails. Early psoriasis can resemble eczema, irritation, or a fungal rash, so appearance alone cannot confirm the diagnosis.

Dryness may lead to fissures when a patch is rubbed or stretched. Scratching can increase soreness and shed scale, while stress, illness, skin injury, or certain medicines may coincide with a flare in some people.

Take a clear photograph when the patch is visible and note where it started, whether it itches, and whether similar areas appear elsewhere. Use a bland moisturiser regularly, avoid picking scale, and keep showers comfortably warm rather than very hot. A clinician can examine the distribution and texture and decide whether testing or treatment is needed.

Treatment depends on the extent and location of the problem. Options may include Topical Corticosteroids, Vitamin D Analogue Treatment, light-based treatment such as Narrowband UVB Phototherapy, or other medical therapy when appropriate.

Arrange an assessment for a new persistent scaly patch, repeated flares, painful cracks, or nail changes. Seek prompt care if large areas become very inflamed, skin breaks are spreading, or joint pain and morning stiffness occur with the skin symptoms. A clinician can also check whether another condition is causing the rash.
