What to try initially

Physiotherapy can assess the hip and lower back and prescribe graded mobility and strengthening. Dry Needling or a Cortisone Injection may be discussed only after an examination identifies a suitable target and other causes have been considered.
When symptoms need prompt review
Seek urgent care for new or worsening leg weakness, loss of bladder or bowel control, numbness around the groin, or rapidly spreading numbness. Arrange prompt assessment after significant trauma, or for fever, unexplained weight loss, severe unrelenting pain, or symptoms affecting walking. These warning signs require evaluation because they are not specific to piriformis syndrome.
