How it can look and feel

Asteatotic eczema, also called eczema craquele or winter itch, often starts on the shins, lower legs, arms, or other areas that lose moisture easily. The skin may look dull, scaly, or ashy and feel tight after bathing. As the barrier becomes less effective, small surface cracks can give the skin a cracked-porcelain appearance, with redness or soreness around the lines.

Cold, dry air, hot showers, harsh cleansers, and frequent washing can worsen moisture loss. Dry skin by itself does not confirm eczema; the pattern, itch, inflammation, and response to care help a clinician make the diagnosis.

Early skin care

Use a gentle, fragrance-free cleanser only where needed, keep showers comfortably warm rather than hot, and pat the skin dry. Apply a thick emollient promptly while the skin is still slightly damp, then reapply whenever the area feels tight. Creams or ointments usually protect a very dry barrier better than a light lotion.

Avoid scratching, rough exfoliation, and fragranced products over cracked areas. If inflammation persists, a clinician may consider topical corticosteroids for a limited course and explain where and how to apply them safely.

When to arrange medical care

Arrange a dermatology or primary-care assessment if cracks are painful, bleeding, spreading, or not improving with regular moisturising. A clinician can check for infection, psoriasis, contact dermatitis, or another cause of scaling and choose treatment for the degree of inflammation.

Seek urgent care for rapidly increasing redness, warmth, swelling, pus, fever, or severe pain. Those changes may signal infection in an impaired skin barrier and should not be managed by moisturiser alone.
