What are the alternatives to microdiscectomy?

Microdiscectomy removes a small piece of disc material that is pressing on a spinal nerve, often to ease leg pain. When strength is preserved and symptoms are manageable, a clinician may suggest time, pain relief, activity adjustment and physiotherapy. An injection near the irritated nerve can sometimes help with pain while a rehabilitation plan continues. Endoscopic spine surgery is another surgical approach using a small access route, but its suitability depends on the disc location, anatomy and the amount of nerve pressure. A surgical microscope is used in some procedures and does not itself determine whether surgery is needed.

Record how pain, numbness and strength change with walking, sitting and sleep. An MRI can help match a herniated disc to the symptoms, but the examination remains part of the decision. Ask whether physiotherapy is safe now and which movements to avoid temporarily. Discuss medicines and injections alongside their possible drawbacks. If symptoms do not settle or weakness develops, ask what finding would make surgery preferable and whether an endoscopic approach could address the same pressure.

Seek emergency assessment for new loss of bladder or bowel control, numbness around the groin or inner thighs, or rapidly worsening weakness in one or both legs. These may indicate cauda equina syndrome. Get prompt medical advice for progressive foot drop, severe unrelenting pain, fever with back pain or symptoms after major trauma.
