What are the alternatives to brachytherapy?

Brachytherapy places a radiation source inside or close to the area being treated. External beam radiotherapy delivers radiation from a machine outside the body and can cover a planned field. Surgery removes a tumour or an organ when that is appropriate; prostate removal surgery is one example. The balance changes with the tumour’s location, size, spread, nearby organs, previous treatment, and overall health. Cervical cancer and womb cancer may use combinations of surgery, external radiation, internal radiation and drug treatment. Eye melanoma and retinoblastoma require highly specialised plans, while prostate cancer decisions may include surgery, external radiation, monitoring or medicines.

Ask the oncology team to compare the purpose of each option: cure, reducing recurrence risk, controlling growth, or easing symptoms. Find out whether treatment is local or affects the whole body, how many visits or procedures are expected, and which nearby organs need protection. A second opinion from a radiation oncologist or disease-specific multidisciplinary team can clarify whether the alternative is genuinely suitable for your diagnosis.

Tell your team promptly about new bleeding, severe pain, difficulty passing urine or stool, sudden vision change, or rapidly worsening symptoms. Seek urgent care for heavy bleeding, inability to urinate, severe breathlessness, or sudden loss of vision. The urgency comes from the symptom itself, not from whether brachytherapy is chosen.

Ask which treatment goal applies, why brachytherapy was suggested, and what external beam radiotherapy or surgery would involve in your case. Could combining treatments improve local control? How would the alternative affect fertility, bowel, bladder, sexual function, or vision? What signs should trigger an urgent call?
