Welts on Skin: What They Mean and What to Do

A single welt may be pale, pink, or red, with a smooth surface and a shape that changes over time. Welts can occur on any skin area and may join together into larger patches. They are different from a bruise because they tend to shift or fade rather than stay fixed in one place.

Welts are a common presentation of hives, also called urticaria. Hives can follow contact with heat, cold, pressure, exercise, an infection, or a medicine, although a clear trigger is not always found. Food or an insect sting can also be relevant, especially when symptoms begin soon after exposure.

Some people also develop deeper swelling around the lips, eyelids, hands, feet, or throat; this is called angioedema. Welts that last in the same spot for more than a day, leave bruising, or feel painful rather than itchy need a clinician's assessment because they may not be typical hives.

Take a photograph when the welts are most visible and note recent foods, medicines, infections, products, temperature changes, and activities. Avoid scratching, tight clothing, hot showers, and any suspected trigger while you arrange advice. A cool compress and loose, soft clothing may reduce irritation.

A pharmacist or clinician may recommend an oral antihistamine treatment for itch and wheals. Do not start, stop, or share medicines without checking whether the choice is suitable for you, particularly during pregnancy or when you take other medicines. Persistent or repeatedly returning welts deserve medical review.

Get emergency help immediately if welts occur with swelling of the tongue or throat, trouble breathing or swallowing, faintness, confusion, or sudden weakness. These symptoms can signal a serious allergic reaction and should not be watched at home.

Arrange prompt medical care for rapidly spreading welts, marked facial swelling, repeated episodes, fever, painful lesions, skin that peels, or symptoms that began after a new medicine. A clinician can examine the pattern and decide whether testing or prescription treatment is needed.

Ask: Could these lesions be hives or another type of skin rash? What changes should make me seek urgent care? Should I avoid a particular medicine, food, product, or activity? Which treatment is appropriate for my age, health conditions, and other medicines?
