Water break before delivery

The membranes may release fluid as a sudden gush or a slow leak, and contractions may not begin immediately. Fluid can continue to appear when you stand or move. Your care team will consider how far along the pregnancy is, whether contractions are present, the fluid’s appearance, your temperature, and the baby’s movements. They may examine you and monitor you to confirm the rupture and decide whether you need hospital care or further observation.

Contact your maternity unit as soon as you suspect a break. Wear a clean pad and record the time, colour, smell, and amount of fluid. Avoid tampons, intercourse, swimming, and inserting anything into the vagina until your team advises otherwise. Keep drinking normally unless a clinician has told you to restrict fluids, and arrange transport according to the maternity unit’s instructions.

Seek urgent help for green or brown fluid, fresh bleeding, fever, unpleasant-smelling fluid, severe or constant abdominal pain, reduced baby movements, or feeling faint. A cord or part of the baby at the vaginal opening is an emergency; call emergency services and lie on your side while waiting. If the waters break before the expected time of birth, prompt assessment is needed because it can be associated with preterm labour.

Ask whether you should come in immediately, how long to monitor for contractions, how to check your temperature if advised, and which symptoms mean you should return urgently. Confirm the after-hours contact number before leaving the assessment.
