Watchful waiting for prostate cancer

Watchful waiting is a deliberate care plan, not an absence of care. It is often considered when cancer is not causing troublesome symptoms, when other health problems influence treatment choices, or when the likely benefit of intensive treatment is limited. The aim is to avoid treatment effects unless they are needed.

Follow-up is tailored to the person. It may include appointments, discussion of urinary or bone symptoms, examination, and tests when the clinical team considers them useful. This differs from active surveillance, which commonly uses closer testing to look for signs that cancer is changing.

Keep scheduled reviews and tell the team about new urinary difficulty, blood, pain, tiredness, weight change, or reduced mobility. Ask who to contact between appointments. Bring an up-to-date medicine list and discuss practical support for pain, bladder symptoms, sleep, mood, and daily activities.

Contact the clinical team promptly for worsening pain, new difficulty passing urine, blood in urine, or a noticeable decline in function. Seek emergency help for inability to pass urine, severe uncontrolled pain, new leg weakness or numbness, or loss of bladder or bowel control.

Ask why watchful waiting suits you, what follow-up will involve, which symptoms should be reported, how pain or urinary problems will be treated, and what would prompt a change in the plan.
