What should you know about viral rash without fever?

Viruses can trigger flat or raised spots, small blisters, or areas of redness as the immune system responds. Some rashes itch, sting or feel tender; others cause little discomfort. Fever may be absent, especially early in an illness, after it has passed, or in a person whose immune response is muted.

A rash may accompany a common cold, COVID-19 or chickenpox. Cellulitis is a bacterial skin infection that needs different treatment, while croup and bronchiolitis mainly affect breathing. The location, texture, timing and new medicines or products used on the skin help a clinician narrow the possibilities.

Keep the skin cool and clean, pat it dry and use loose, breathable clothing. Avoid scratching, fragranced products and any cream that has previously caused irritation. A cool compress may soothe itch. Do not share towels, clothing or bedding if an infectious rash is possible, and wash hands after touching affected skin.

Take a clear photograph in good light and record when the rash began, where it started, how it has changed, and any new foods, medicines, detergents or contacts. Do not pop blisters. A pharmacist or clinician can advise on an age-appropriate treatment for itch.

Get emergency help if the rash does not fade when pressed and the person is very unwell, confused, unusually sleepy, struggling to breathe, swelling around the mouth, or has a seizure. Urgent assessment is also needed for rapidly spreading redness, severe pain, skin peeling, extensive blistering, or involvement of the eyes or mouth.

Arrange a clinical review when the rash is spreading, infected-looking, intensely itchy, recurrent, or not improving. Seek advice promptly for a young infant, pregnancy, or a weakened immune system. Do not rely on the absence of fever to judge seriousness.
