What should you know about very bad nappy rash?

The skin may be intensely red, raw, swollen, shiny, or broken, with soreness during cleaning or passing urine or stool. Irritation from moisture, friction, and contact with urine or stool is common. A rash that involves the skin folds, has small surrounding spots, or continues despite careful care can have a yeast component. Blisters, pus, crusting, rapidly spreading redness, or marked tenderness need prompt clinical assessment because infection or another skin condition may be present.

Change the nappy promptly after wetting or soiling. Rinse with lukewarm water or use a fragrance-free cleanser, then pat rather than rub. Allow short periods without a nappy when practical, and apply a thick protective cream at each change. Avoid fragranced wipes, talc, scrubbing, and home remedies that sting. A clinician may recommend emollient therapy for the damaged barrier or topical antifungal treatment when yeast is suspected; use medicines exactly as directed.

Arrange medical advice promptly for broken or bleeding skin, blisters, pus, fever, unusual sleepiness, poor feeding, significant pain, or redness that is spreading. Seek urgent help if the child seems seriously unwell or is difficult to wake. Contact a clinician when the rash is severe, keeps returning, or does not begin to settle after careful nappy care.
