What should you know about vaginal diaper rash?

Vaginal diaper rash usually affects the external skin around the vulva, where moisture, friction, stool or irritants can cause redness and soreness; gentle cleansing, frequent nappy changes and a protective barrier can help. The rash is on the skin, not inside the vagina. In a baby, redness may involve the vulval folds, buttocks, groin or nearby skin. A clinician should assess a rash that is severe, spreading, blistered, infected-looking or not improving with gentle care.

Nappy skin stays warm and damp, and contact with urine or stool can weaken its outer barrier. Rubbing from a tight or wet nappy then makes irritation more likely. Fragranced wipes, soaps, bubble baths and laundry products can add to the inflammation. A rash that is bright red and extends into the skin folds, with small separate spots around its edge, may be a yeast-related rash rather than simple irritation. Other conditions can resemble nappy rash, including eczema, bacterial infection or a skin reaction to a product, so appearance alone cannot confirm the cause.

Change the nappy promptly after it becomes wet or soiled. Rinse the area with lukewarm water or use fragrance-free, alcohol-free wipes, then pat dry rather than rubbing. Allow short periods without a nappy when practical, and fasten each fresh nappy comfortably rather than tightly. Apply a thin, even layer of a bland protective barrier to clean, dry external skin; an emollient may support the skin barrier. Avoid scrubbing, talc, antiseptics, perfumed products and home remedies placed inside the vagina. Do not use a steroid or antifungal cream unless a pharmacist or clinician has advised which product is suitable and how to use it.

Arrange medical advice if the rash is painful, rapidly worsening, recurrent, or fails to start settling after a few days of careful nappy care. Seek prompt assessment for broken skin, blisters, pus, bleeding, marked swelling, fever, unusual discharge, pain when passing urine, or redness spreading beyond the nappy area. A very young or unwell baby needs timely advice. Bring the products used on the skin and describe recent changes in nappies, wipes, soaps or medicines. A clinician can distinguish irritation from yeast, bacterial infection or another condition and recommend treatment when needed.

Ask: Does this look like irritant diaper dermatitis or another condition? Should I use an emollient or a topical antifungal treatment? Which products should I stop, and how should the area be cleaned? How soon should the skin improve, and what changes would require urgent review? If the rash keeps returning, could a nappy fit, product sensitivity or another skin problem be contributing?
