UTI and hives: what to know

Hives are raised, itchy welts that can appear and fade in different places. A urinary tract infection can occur at the same time as hives, but the rash may also be triggered by an antibiotic, pain medicine, another exposure, or a separate viral illness. The presence of hives does not identify the cause of urinary symptoms.

Note when the itching or swelling began relative to the infection and every medicine dose. Taking photographs of changing welts can help because individual hives may disappear before an appointment. Swelling of deeper skin, especially around the lips or eyelids, can occur with angioedema.

Contact the clinician treating the UTI promptly if hives began after a new medicine. Do not restart a suspected medicine or change prescribed antibiotics without advice; the infection still needs appropriate treatment. A clinician may recommend an oral antihistamine for itch and welts, taking account of your age, pregnancy status, other medicines, and health conditions.

Use cool compresses, avoid hot showers and tight clothing, and do not scratch. Keep a list of medicines, supplements, foods, and recent products. A rash with urinary symptoms may need separate evaluation rather than being treated as a simple drug reaction.

Call emergency services immediately for trouble breathing, wheezing, throat tightness, faintness, or swelling of the tongue or mouth. Seek urgent medical care for rapidly worsening skin swelling, blistering or peeling, fever with a very unwell feeling, or a rash involving the eyes or mouth. Persistent hives should be reviewed, particularly if they continue after the suspected trigger is removed.

Share the UTI symptoms, medicine names, dose timing, photographs, and any prior drug reactions. Ask whether an antihistamine is suitable, whether the antibiotic plan needs changing, and which symptoms would require urgent reassessment.
