Urinary urgency without a UTI: what to know

A negative urine test does not mean the symptom is imaginary; it means a bacterial urinary infection has not been shown by that assessment. Urgency can occur with an overactive or irritated bladder, constipation, pelvic floor tension, bladder pain conditions, high caffeine intake, or medicines that increase urine production. Incomplete emptying can create repeated urges, particularly with weak flow or straining. Prostatitis or benign prostatic hyperplasia may contribute in people with a prostate. The appropriate explanation depends on the full pattern.

Keep a bladder diary with timing, drinks, urgency, volume if practical, leakage, pain, bowel habits, and flow changes. Spread fluids through the day, reduce caffeine or alcohol if they trigger symptoms, and avoid repeated preventive trips to the toilet when it is safe to wait briefly. Address constipation and review medicines with a clinician. Do not keep taking antibiotics after a negative assessment unless a prescriber has advised it.

Arrange review if urgency persists, disrupts sleep, causes leakage, or affects work and travel. Seek prompt care for fever, back or side pain, visible blood, severe pain, vomiting, inability to pass urine, or new weakness or confusion. A clinician may repeat testing, assess bladder emptying, review medicines, and consider bladder, pelvic, bowel, or prostate causes.
