What these symptoms can mean

Upper back pain can come from muscles, joints, posture, nerve irritation, or an injury. Extreme tiredness may follow poor sleep, infection, emotional strain, anaemia, medicine effects, or another health problem. When cancer is present, fatigue can also accompany the illness or its treatment; some blood cancers, including leukaemia, lymphoma, and myeloma, can present with fatigue, while pain may have a separate cause.

The combination does not identify one condition. A clinician will ask when it began, whether it is worsening, what makes it better or worse, and whether there are fever, weight change, breathlessness, bruising, night sweats, weakness, or bowel changes.

What to do now

Arrange an appointment if the symptoms persist, interfere with sleep or daily tasks, or keep returning. Record the pain’s position and character, your energy pattern, temperature, sleep, medicines, and any associated changes. Gentle movement may help some muscular pain, but avoid activities that sharply increase symptoms. Do not start supplements or pain medicines regularly without checking whether they are suitable for you.

Your clinician may examine your back and nerves and decide whether blood tests or imaging are appropriate. If you already have cancer, tell the oncology team about a new pain pattern or fatigue that is different from your usual treatment effects.

When to seek urgent care

Seek emergency help for upper back pain with chest pain, severe breathlessness, fainting, sudden weakness, new loss of bladder or bowel control, or numbness around the groin. Get urgent assessment for fever with a very unwell feeling, rapidly worsening pain, a recent significant injury, or new leg weakness. These warning signs need prompt evaluation and should not be judged by how likely cancer seems.
